Golf diary.

When I was at Kooralbyn I was given a AGU jnr golf diary. I found this very helpful as I could record my rounds and stats and such. Try as I may I am unable to source one of these or something similar. I do not want an electronic version as there are things I'm happy to spread but sheets are not one of them. Any help would be appreciated.

Didn't titleist have something similar a couple of years ago? I may have one at home still, but won't be back until mid next week.
